Please come in at least 10 minutes before your appointment time so that we may enter your information and have your pet in with the doctor in a timely manner. To save time, you can download and print a new client form, fill it out, and bring it in with you for your first appointment.
We ask that you contact your previous veterinarian and bring those records with you or request that they fax these records to us. Our fax number is (772) 334-7447. Be sure to let us know that the records will be faxed to us. For new puppies and kittens, we ask that you bring all information that you received with your new pet (health certificate, vaccination records, etc.).
Our boarding facility is equipped with small, medium, and large runs as well as small animal cages. Keeping your pet(s) comfortable while staying at Jensen Beach Animal Hospital is very important to us. You might find this Checklist helpful in preparing for your pet’s stay.
All surgical patients should be admitted between 8am and 9am on the day of their scheduled procedure. Typically, patients will stay for the majority of the day to ensure proper monitoring after anesthesia. We do require that all surgical patients be fasted 10-12 hours before surgery. Procedure estimates are available upon request and we would be happy to schedule surgical consultations prior to the day of surgery if you have any questions or concerns regarding your pet’s procedure.

Waiver – Release Records – Please Print
To have your records sent to another clinic or if you would like a copy of your pets records, we must have a signed copy of this form. Please print and be sure to fill the waiver out completely. You may drop the waiver off at our office, email the waiver to jbahvet@gmail.com, or fax to (772)334-7447. All records request will require a minimum of 24 hour notice and we will do our best to get the records processed as quickly as possible. Should you need to expedite this process for any reason, please feel free to contact our office directly.
